HeadSpa Liability Waiver and Consent Form

I hereby acknowledge and agree to the terms set forth in this HeadSpa Liability Waiver and Consent Form (“the Agreement”) in consideration of participating in the HeadSpa services provided by Aroma Head Spa & Massage.

1. Assumption of Risk:

I understand that HeadSpa involves various activities, such as head and scalp massages, which may carry certain inherent risks. I acknowledge that these risks include, but are not limited to, potential physical injuries, allergies, or reactions to any products used during the session. I voluntarily assume all risks associated with participating in the HeadSpa services.

2. Health and Medical Condition:

I confirm that I am in good health and do not have any medical conditions, injuries, or allergies that may prevent me from safely participating in the HeadSpa. If there are any changes to my health condition, I will inform the HeadSpa staff or manager.

3. Release of Liability:

In consideration of being permitted to participate in the HeadSpa services, I hereby release, waive, discharge, and hold harmless Aroma Head Spa & Massage, its owners, employees, contractors, and representatives from any and all claims, liabilities, expenses, or damages that may arise from my participation in the HeadSpa services, whether caused by negligence or otherwise.

4. Indemnification:

I agree to defend, indemnify, and hold harmless Aroma Head Spa & Massage, its owners, employees, contractors, and representatives from any and all claims or demands, including attorney fees, arising from my participation in the HeadSpa services or any breach of this Agreement.

5. Consent:

I understand and agree that the HeadSpa services provided by Aroma Head Spa & Massage may involve the use of various products and techniques. I hereby consent to the use of such products and techniques during my HeadSpa session, understanding that I have the right to ask questions or seek further information regarding any aspect of the services provided.

6. Personal Property:

I understand and agree that Aroma Head Spa & Massage is not responsible for any loss, theft, damage, or misplacement of my personal belongings while at the HeadSpa premises.

7. Reservation Cancellation Policy:

I am aware of Aroma Head Spa & Massage’s reservation cancellation policy and agree to abide by it. Any changes or cancellations must be made within the specified time frame, as communicated by the HeadSpa staff and written within the booking website.

8. Release Liability for minor (at least 12 years of age):

If I am executing this Release of Liability and Waiver of Rights on behalf of a minor (at least 13 years of age), I warrant and represent that I am the minor’s parent or legal guardian.

9. Medical Assistance:

In the event of an emergency, I hereby give Aroma Head Spa & Massage Staff permission to call for medical aid for myself or the minor listed below, and to transport the latter to a hospital or medical institution. In addition, I commit to paying for any and all associated costs of any such medical care and/or related transportation, and I thus indemnify and hold the Released Parties harmless from any and all such costs.

10. This Agreement shall be binding on my (or the below-mentioned minor's) estate, heirs, administrators and assigns
